What is Cap-C?

The Community Alternatives Program (CAP-C) is a North Carolina Medicaid waiver program that provides home and community based services to medically fragile children as an alternative to institutionalization or facility placement. These services allow the beneficiary to remain in their home and assists in supplementing their natural and informal supports with formal supports to allow for the maintenance of their health, safety and well being.

How We Can Help

Through the CAP-C Program we are able to assist medically fragile children (ages 0-21) and their families in the surrounding counties of Southeastern North Carolina. Our beneficiaries meet the same level of care criteria as is needed for institutionalization and have a desire to receive services and supports in their home and community environments as an alternative. As case managers and care advisors for CAP we specialize in facilitating approved waiver services as well as additional non-waiver services and supports to achieve this goal of community integration.
